Since 1988, Daybreak has been providing comprehensive case management services to adults seeking to live a complete and largely self-sufficient lifestyle in the Mat-Su Valley. Our initial focus was working with those with a diagnosed mental illness, helping them to engage in their communities by assisting them to find and obtain employment, housing, transportation and other services. Being diagnosed with a mental illness can be difficult at any age, and many Alaskans aren't sure where to turn for help for themselves or a family member. At Daybreak, we're here to help those who have received a diagnosis to live a fulfilling life through comprehensive, independent case management services. Our philosophy of care includes a belief that such a diagnosis doesn't mean an end to a joyful life, and that recovery from mental illness is possible.

Daybreak, Incorporated is currently recruiting for two entry level case management positions at our Palmer office.

 As a member of the Daybreak case management team, know that you will be working with a team of professionals who are committed to your success in the workplace as well as the success of every consumer we work with. We work from a strengths perspective to assist the consumer in building on their own strengths to develop recovery plan that is individually tailored to meet their mental health recovery needs.

Daybreak case managers assist adults, who have been diagnosed with long term mental illnesses to create an individualized treatment plan that:

  • Connects the consumer to mental health, medical and other therapeutic services
  • Improves independent living skills in the community
  • Assists in locating and maintaining safe, affordable housing
  • Navigates social security, public assistance, and Medicaid benefits
  • Improves overall wellness

This position requires the minimum qualifications:

  • A bachelor's degree from an accredited college or university (social work, human services preferred) or three years' experience working in the social services field.
  • Successful completion of a State of Alaska DHSS background check
  • Strong written and oral communication skills
  • A valid Alaska driver's license
