Job Description Title: AL Resident Care Coordinator
FLSA Status: Exempt Reports to: Resident Services Director
Hire, train, supervise and schedule care staff members as well as coordinate all services relating to resident care needs in AL unit (and MC unit as needed or as determined by the supervisor or Clinical Corporate Team). Manage resident changes of condition in coordination with Resident Service Director. Audit the medication administration program to assure accuracy and completeness. Address resident and family concerns, respond to resident special requests, and act as a liaison within the community with other staff.
Essential Functions:
- Participates in the hiring and training of clinical staff in the assisted living neighborhood.
- Provides in services and staff meetings to inform clinical staff and keep up to date of any new changes and to assure quality of care for our residents.
- Scheduling and Appropriate staffing of care staff in collaboration with the RSD and ED. Constructs and provides a schedule for the assisted living clinical care staff as assigned, making sure there is coverage on every shift (and supports MC neighborhood as needed). All overtime hours must be pre-approved by the RSD/ED.
- Develop the initial monthly clinical schedule for each community and ensure all shifts are filled for the month. Have this developed and posted at least 10 days prior to start date.
- Review clinical applications for Resident Service Director to ensure Medication Aide applicants are certified. Provide these applications to the RSDs at each community based on needs.
- Review staffing patterns monthly with Executive Directors and makes adjustment based on census.
- Provides care staff coverage as needed when unable to fill a shift when a call out arises
- Maintains attendance records, track and log attendance for all clinical staff.
- Participate in all in-service programs to expand and develop abilities.
- Maintains compliance to all personnel policies, established community policies and procedures, and Federal and State regulations and standards, including but not limited to HIPAA privacy and confidentiality laws.
- Provides direct resident care as needed based on the needs of the community. Functions as a caregiver when assigned including monitoring and responding to resident call response system.
- Develops and maintains courteous and professional relationships with residents, visitors, and co-workers.
- Dependable and able to meet schedule requirements (attendance and punctuality) with no restrictions.
- Coaches and counsels employees as needed and exercises disciplinary action by oral and written communications with review and approval of Resident Services Director.
- Attends daily stand up meetings in absence of the Resident Services Director with other managers and Executive Director with 24-hour report any incident reports from the previous day/evening/night shifts, and Medication Exception Report.
- Monitors all residents making sure any change in conditions are reported to the Resident Services Director and the resident's primary care physician, charting in the resident's chart, as well as reviewing all new physician's orders making sure medications are correctly transcribed and added to the MAR/EHR system and that the medication is obtained in a timely manner.
- Reviews Medication Exception Report every morning to ensure all medications ordered were given by the medication aides/technicians. Coordinates care with third party agencies by having weekly care conferences with home health and hospice providers.
- Assists with formulation of a service plan for each resident assuring input from family and staff, which accurately describes the care of each resident that is individualized and concise after performing a detailed assessment.
- Assists in the new resident move-in process by making sure all documents are received and evaluations are completed. (Ambulation and Mobility, Elopement, Smoking, Unsupervised Absence)
- Assures all admitting physician's orders are on hand, H & P or equivalent (as per each state regulation) is completed by physician prior to move in.
- Assures a diet is ordered and communicated to the chef and kitchen, as well as any food allergies, to assure proper diet is prepared for the new resident.
- Communicates to care staff the new resident's needs and preferences by providing service plan for review and New Resident Move-in Notice.
- Reviews all medication orders daily and follows up on medications that were not preauthorized in resident's Service Notes.
- Reports any incidents to the Executive Director and Resident Services Director.
- Establishes and monitors an at-risk report identifying any residents and reporting to Executive Director and Resident Services Director.
- Performs QA audits of resident's charts, medication carts, task summary sheets, medication administration records as needed to ensure state compliance and accuracy.
- Monitors all ordered supplies.
- Coordinates resident appointments when required.
- Performs additional Quality Assurance Audits as directed.
- Provides input for enrichment programs/ options.
- Conducts/ coordinates the delivery of ongoing monthly support groups for resident family members.
- Other duties as assigned.
Non-Essential Functions:
- Attends all required training, in-service, and staff meetings.

- Maintains a positive and professional demeanor toward residents, visitors, families, and co-workers.
- Adheres to all policies and procedures of Aventine Hill Communities.
- Performs other duties as assigned by the Resident Services Director.
Qualifications/Skills/Requirements |
- Currently certified and willing to maintain CPR Certification and First Aid.
- Recent history of care experience in assisted living setting.
- Knowledge of medication administration or possess MT experience in assisted living setting.
- Excellent customer service and communication skills.
- Positive and cheerful disposition.
- Excellent assessment skills.
- Proven leadership and organizational qualities.
- Comfortable making decisions and directing others and resolving conflict.
- Knowledge of progression and care of dementia residents.
